We exist to support individuals with mental health challenges who need assistance connecting to resources such as access to medical, social, or educational services, housing, financial aid, court appearances, disability, SSI, or other essential services to meet basic needs and maintain independence within the community. We work with children aged 2 to 17, adults 18 and older, and their families.
Services include:
. Initial assessment and analysis.
. Home visits.
. Connection to appropriate services.
. Advocacy for their rights.
. Referrals to community resources.
. Follow-up on ongoing services.
. Each beneficiary receives support from a case manager, supervised by a skilled and experienced team.
